• Your Role : To provide expert clinical assessment and interventions as part of a dynamic, multidisciplinary team that reacts quickly when
people are in crisis. Our target response time is under 2 hours. We provide ongoing hospital level care, supported by Advanced Clinical
Practitioners (ACPs) and Consultants.
• Your aim: To help patients stay in their own home, empowering recovery and preventing unnecessary conveyance or hospitalisation.
• Your team : Add your expertise to our multi-disciplinary community team working with Physiotherapists, Nurses, Paramedics, Advanced
Clinical Practitioners, Consultants and Healthcare Assistants. Our team works closely with Intermediate Care staff to provide support to
keep people at home wherever possible

Main duties of the job
Your main duties include:
• Comprehensive specialist assessment, review and evaluation of the needs of patients and carers within a 0-2 hour, 2-24 hour timeframe
• Being responsible for undertaking an urgent community response to people in their own homes to prevent hospital conveyances and
keeping patient safe at home with the appropriate clinical interventions.
• Working with a range of professionals in order to ensure care is appropriate and delivered in a timely manner.
• Demonstrating professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
10. Registered Healthcare Professional with HCPC Nurse Degree/ Diploma in Nursing
11. Experience of recent Continuous Professional Development
12. Relevant post graduate clinical training e.g. Advanced History Taking & Assessment
13. Training in: Long Term Conditions Respiratory Disease Cardiac Disease Trauma, Orthopaedics & End of Life
EXPERIENCE
Essential criteria
14. Demonstrate experience post or pre registration placements in relevant clinical areas for therapists:- Care of Elderly Orthopaedics Neurology Respiratory Palliative Care Long term conditions
15. Experience of working in an NHS community/in-patient setting for a minimum of 2 years post registration
16. In depth experience of working within multi-agency/multi-disciplinary team
17. Experience of working with NHS or Adult Social Care single assessment process
Desirable criteria
18. Experience of electronic clinical systems
19. Experience participating in research or audit project
20. Experience of day-to-day staff management e.g. case allocation to Health Care Assistants

SPECIAL CIRCUMSTANCES
Essential criteria
33. Valid and current UK driving licence and car with Business Insurance to be able to travel to locations throughout Buckinghamshire
