We are seeking a highly skilled and compassionate Advanced Nurse Practitioner (ANP) to join our team across four GP practices within a three‑mile radius in the Tameside area.
The successful candidate will deliver comprehensive patient care, undertake clinical assessments, and support effective clinical decision‑making. We provide one‑to‑one training, weekly tutorials and an on‑call GP for guidance.
Both full‑time and part‑time positions are available. Candidates must be able to work until 18:30 and remain later if necessary.
Main duties of the job
* Conduct thorough patient assessments, including medical histories and physical examinations
* Provide home‑visiting service for all four sites – covering a three‑mile radius in the Tameside area
* Develop and implement individualized care plans in collaboration with patients and multidisciplinary teams
* Prescribe and administer medications safely in line with established protocols and guidelines
* Monitor patient progress and adjust treatment plans as required
* Provide health education and counselling to support patient understanding and self‑management
* Accurately document all clinical interactions within electronic health records
* Ensure compliance with all legal, ethical, and organisational standards relating to patient care
Qualifications
* Registered Nurse with current NMC registration
* MSc in Advanced Clinical Practice or Advanced Nursing Practice (or equivalent)
Experience
* Experience of working in a clinical setting with evidence of autonomous practice
* Experience of lone working in a home‑visit setting
* Ability to make independent decisions
* 3–5 years of A&E/Urgent Care at Staff Nurse Level or above
* 2 years of independent prescribing
