First Episode Psychosis Care Co-ordinator

Cardiff and Vale early intervention service called Headroom has developed in line with the NICE and NHS Cymru guidance. The Early Intervention in Psychosis Service provides early detection, assessment and intervention for 14-25 year olds who are experiencing a first episode of psychosis.
The post holder will deliver evidence based holistic care to clients within the context of a multi-disciplinary, multi-agency community mental health service.
Main duties of the job
Take responsibility for a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Early Intervention Service in accordance with Parts 2 and 3 of the Mental Health (Wales) Measure 2010.
Foster robust working relationships with all partners and key stakeholders, in particular Local Authority, Primary Care and the Third sector.
Contribute to the continuing improvement and development of community mental health services.

Person Specification
* Registered Mental Health Nurse, Occupational Therapy or Social Work.
* Evidence of post-registration continuing professional development activity.
* Evidence of experience in clinical practice within the area of speciality e.g. adult or children and young peoples' mental health.
* Qualification in psychological and psychosocial interventions for psychosis.
* Experience of working within a multi-agency community mental health setting.
* Experience of working with 14-25 year olds with psychosis.
