Within the Access Team is the crisis team, Hospital Liaison Team and Single Point ofAccess. This role will require you to work across all three of the areas. Youwill also be joining the team at an exciting time in Navigo Acute services asthings are changing and developing to ensure we continue to deliver outstandingservices to the population of North East Lincs. Main duties of the job
The Access Team Practitioner will work across the Crisis Team, HospitalLiaison Team and Single Point of Access. You will have responsibilitiesincluding initial assessment, implementation, planning, and evaluation of care.Assessment of risk along with subsequent management and safety planning is asignificant part of this role- face to face and over the telephone. You will work on a 24 hour rota whichincludes weekends and nights. You willact as site lead out of hours which includes overseeing and supervising otherareas of acute services when required.The role will entail both team working and autonomous working practices,together with personal responsibility for caseload management from firstcontact to discharge. You will berequired to manage and supervise junior staff and students, also delivertraining/information to the team and wider services when required. The Post Holder will work closely within themulti-disciplinary and inpatient team along with carer/family.

Job responsibilities
To develop and take an activerole in quality monitoring of the service, including leading of researchprojects and audit activity with the aim of improving the service/servicearea. Partnership with service users andcarers is integral to these activities.
The Post Holder will be ableto evaluate and review research and other relevant literature and beware of theimplications of the social and local policies, procedures and informationpackages related to service delivery.Will have the ability to work under pressure and manage staff andservices
Management and Leadership
The Post Holder will act as arole model for RETHINK staff, which will involve providing support and guidanceto RETHINK staff in order to maximise the experience of clients whilst they arestaying at Fieldview.
The Post Holder will workwithin an environment that involves taking responsibility for the use of asignificant number of resources, including four crisis beds, access toin-patient services, processing of referrals and allocation of activity toachieve rapid response to crisis referrals.
Will plan and organiseworkloads for staff within the area to include allocation of staff, planningpatient care and groups etc.
The Post Holder will supervisethe day to day care of their service area, ensuring staff are designated toappropriate areas. To also be involvedin the recruitment and selection of staff along with appraisals and supervision.
Education and Development
Acute Mental Health is alearning environment which accepts short-term placements, any qualifiedpractitioners and a wide variety of other agencies, wishing to gain an insightinto the Mental Health Services.
There is an expectation thatthe Post Holder will be involved in the provision of internal training sessionsand presentations to other services and students.
The Post Holder may be askedto provide specific training or mentorship to students.
The Acute Mental Health Teammembers are expected to become actively involved in the change process anddemonstrate commitment to changing role, i.e. in the development of specialistprotocols and specialised programmes of care.
Team members will providecontinuous development and evaluation of patient information packages to theservice.
Individuals within the teamwill provide teaching and education to services users and carers with regard toliving with mental ill health, and how to access service providers. This will involve working closely with othercare providers, including relatives in a constructive manner using education,advice and support.
The Post Holder will beexpected to organise and manage their own clinical supervision, and supervisepeers, within Navigo agreed guidelines.
Clinical Patient Care Dutiesand Responsibilities
To provide high quality carein either the home, Fieldview or hospital setting. Clients referred to the service will usuallyhave complex needs and problems.Families/carers will also need appropriate support and there will beregular liaison/referral to other agencies.This will require capability in the following areas:
1.To maintain accuraterecords, both written and computerised to a standard that reflects their levelof clinical expertise. This may involvethe use of specialised psychometric tools to complement the assessment andevaluation of client care.
2.The Post Holder must haveexperience of using psychosocial interventions in clinical practice.
3.Liaison with community basedservices and resources such as education, Social Security, employment, housing,Police and voluntary sector organisations to facilitate the delivery of themost appropriate and best possible care for the individual client, carers and their families.
4.Discuss complex issuessurrounding risk management and plan care accordingly advising others regardingmanagement of care.
6.To respond to client crisisin a positive pro-active manner.
7.To develop clear carepathways, including methods to reduce the risk of further crisis developing.
8.To work in a manner which isflexible according to client need.
9.To provide telephone supportto any clients, carers, outside agencies or clinicians who require information,advice or crisis intervention. This willrequire the Post Holder to make immediate, effective decisions that will haveimplications for the outcomes of the clients mental health.
10.Be able to assess complexmental health conditions, then to develop and implement care packages in avariety of settings.
11.If required (or at theirrequest) undertake Mental Health Act Training (AMHPs) and if qualified takepart in the rota.
12.To assess if required inthe Police Station.

Qualifications
* A current professional qualification (RMN/RNMH/SW or equivalent.)
* Ability to demonstrate clinical leadership in a multi-disciplinary team setting
* Evidence of post graduate training / studies
* Post qualification award in health/social care at degree or equivalent level
* Clinical Supervision Training
* Nurse prescribing
Experience
* Two years post registration level
* Experience working with mental illness of a severe and enduring nature in an acute or crisis setting
* Knowledge of relapse prevention.
