We have an exciting job opportunity for a dedicated Band 5 Nurse to join our friendly and progressive Community Children's Nursing Team in Ealing. Our team is well-established and committed to providing compassionate, family‑centered nursing care to children who are facing acute, complex, and long‑term health conditions. We provide a 5‑day service to children and families living in Ealing Borough, supporting palliative and end‑of‑life care. Join us and become an integral part of our team.
Responsibilities
Children’s Community Nurses work closely with people who use our services and play an important role in helping children and young people receive the care they need while empowering families to manage their health effectively within the community setting.
* Keep people who use our services as safe as possible through the use of sound clinical skills and effective risk assessments.
* Ensure the best possible clinical outcomes by using up‑to‑date skills and adhering to evidence‑based policies and procedures.
* Ensure people using our services have a good experience by respecting, empowering, and working in partnership with them throughout the care planning process.
We believe that the best health care is delivered by multi‑disciplinary teams working well together and in partnership with other teams and services to provide seamless care. Children’s Community Nurses work as part of a team and you will contribute to the success of the team by being an effective role model and supervising HCAs, to ensure they are working to the standards that the people using our services deserve.

Person Specification
Education & Qualifications
* RSCN / RN(Child) NMC registration.
* Evidence of post‑registration education and training, including paediatric training.
* Education to degree level.
Experience
* Evidence of post‑registration experience.
* Recent acute general children’s nursing experience.
* Experience working with multi‑disciplinary teams.
Knowledge
* Knowledge of Nursing and Midwifery Council Code of Professional Conduct.
* Knowledge and experience of Personal Development Planning.
* Knowledge and demonstration of the use of evidence‑based care.
* Knowledge and experience of clinical supervision.
* Knowledge of equal opportunities and their impact in practice.
Skills & Abilities
* Able to demonstrate recent use of clinical skills, including palliative care, respiratory, cardiac, gastroenterology, special needs and complex care and/or life‑threatening/limiting conditions.
* Ability to work independently and within a team.
