Are you an experienced palliative care nurse with a passion for innovation and improvement? Marie Curie is looking for someone highly driven and enthusiastic about palliative care improvement to join our new Primary Care Network (PCN) project in Exeter. You will support proactive and reactive identification and support of palliative patients in community settings and assist in local ED discharge of end‑of‑life patients.
What is our PCN Project?
This project is a new opportunity to reform and influence Exeter’s end‑of‑life care, focusing on the link between identification, deprivation and admission. With a strong focus on admission avoidance and early identification, Marie Curie nurses will be embedded in GP practice networks, empowering a “why not home?” approach and striving to uphold patients’ preferred place of death.
What Is The Role
We are seeking a skilled Clinical Nurse Specialist in palliative care to develop and enhance the service, primarily improving the identification of patients needing palliative and end‑of‑life care through advanced clinical assessments and cross‑disciplinary collaboration. The role requires confidence, clinical proficiency, person‑centred care, and the ability to expand access to underserved communities.
Key Responsibilities
* Deliver compassionate care and set a strong example for colleagues.
* Lead advanced care planning with patients and families.
* Work closely with the wider PCN team to improve patient outcomes.
* Develop and implement best practice initiatives.
* Coordinate patient activity and ensure high standards of care.
* Ensure effective communication with patients, families, and the wider community.
* Participate in clinical governance, audit, and quality improvement activities.
Job Structure
Permanent; Full time 37.5 hours per week
Shifts
Day shifts; Monday to Friday
Salary
Marie Curie Pay Scale Band 6 (Aligned with AFC) £39,959 – £48,117 per annum

* Experience in specialist palliative care with proven ability to provide comfort and quality of life during terminal illness.
* Experience in advanced care planning and end‑of‑life discussions.
* Registered Nurse on the NMC register (adult) with adherence to professional standards.
* Ability to connect emotionally with patients and families.
* Demonstrated research, data collection, and case study skills to support project delivery.
* Commitment to full‑time hours Monday to Friday.
