Requirements:
* Conduct assessments under the Care Act 2014 and the Mental Capacity Act 2005, working directly with individuals, families, and professionals.
* Take charge of Care Planning and offer short-term case management to adults requiring care and support.
* Participate in Multi-Disciplinary meetings and manage initial Safeguarding enquiries.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
