Overview
Join to apply for the District Nurse role at Midlands Partnership University NHS Foundation Trust. This is a part-time position (30 hours per week) across a 7 days per week service, with the ability to travel independently across the geographical patch. Flexible working requests will be considered.
We are looking for a NMC Registered Band 6 District Nurse with a Specialist Practitioner Qualification in District Nursing who is dynamic, motivated and skilled to support, coach and develop individuals to deliver high-quality, person-centred care to patients within their own homes.
You will be supported by our Lead Specialist Practitioner and Community Nurse Educator.
Responsibilities
* Manage a caseload of patients and provide supervision for team members and students to enable high-quality care in line with MPFT policies and targets.
* Deliver high standards of care within the home setting and contribute to service development and improvements.
* Develop and coach staff to deliver high-quality services and provide leadership as appropriate.
* Carry out detailed professional and clinical assessments and implement and evaluate care/support plans.
* Undertake delegated tasks in team management in collaboration with the Operational Lead.
* Coordinate onward referrals to other MDT members, including Specialist Pelvic Health, Community Matron, Diabetes Specialist Nurse and therapy services.
* Communicate with GPs and Palliative Care teams to ensure integrated care.
Essential Qualifications and Experience
* NMC registered Band 6 District Nurse with Specialist Practice Qualification in District Nursing (SPQDN).
* Working towards SPQDN qualification (if applicable).
* Experience as a SSN/Band 6 in community settings.
* Caseload management and community experience.
* Palliative care management experience.
Desirable Qualifications
* V150 Prescribing
* V300 Prescribing

* UK Registration with current professional status is required.
