The post holder will be responsible for the care and management of children with diabetes, providing specialist advice for families and other health care professionals following diagnosis in both the acute and community settings.
Responsibilities
* Providing enhanced clinical knowledge and skills, work in partnership with acute, community and primary care colleagues to provide a quality service to children and young people with diabetes.
* To organise, manage and prioritise own caseload. Use initiative in the management of children and young people with diabetes within parameters of established and agreed procedures.
* Communicate with children, young people, carers and families in such a way that empower them to make informed choices about their health which will improve their health outcomes.
* To co-ordinate the development, implementation and evaluation of nursing care plans in consultation with the child and his/her family, working in close co-operation with other disciplines involved in the child’s care.
* To provide support, education and counselling to families/carers whose child has been diagnosed with diabetes, by working in partnership with parents to promote the welfare of the child.
Skills and Qualifications
* Registered Nurse (Children’s) on Part 1 of the live NMC Register.
* Children’s Diabetes Qualification (short course in diabetes) OR willingness to undertake.
* 2 years post registration nursing experience at Band 5 (excluding courses) and which must include the provision of diabetes assessment / treatment and management in children & young people.
* Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment. This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approve
* Ability to work flexible hours as the needs of the services demands including evenings, nights, weekends, bank holidays and on-call.
