An exciting opportunity has arisen to be part of the Collaborative Care Home support team in NHS Highland to improve the lives of those who reside in formal 24 hour day care settings.
We are seeking a motivated Occupational Therapist with excellent communication skills and experience to work with stakeholders and the MDT, to provide a coordinated approach aimed improving the experiences of care home residents.
With promotion of a rehabilitation informed approach, the post holder will also be a point of contact for care homes, deliver a comprehensive training package to support the sector as well as set up direct access to specialist rehabilitation.
Working in collaboration with third sector and internal providers, the project manager for registered services and other health care professionals, you will help develop the work plan as an MDT to address the deliverables set out by the Scottish Government.
