Were looking for a proactive Care Team Leader/Support Worker to support a vibrant 25-year-old man with physical and developmental needs.Our client thrives in an environment full of activity and engagement. He communicates by a variety of movements and facial expressions.
He experiences epileptic seizures which vary greatly and require constant monitoring. He is wheelchair dependent, living in a fully adapted home with his family, he requires 2:1 support for all his personal and daily care needs (he cannot be left unattended). This is a unique opportunity to be part of a close-knit team that works collaboratively to enhance his quality of life through meaningful interaction, therapeutic activities, and structured routines.Main Duties and ResponsibilitiesAs Care Team LeaderLead and support a team of Support Workers (including supervising staff)Plan rotas, oversee absence and annual leaveUpdate care plans, risk assessments and maintain accurate care recordsMonitor staff competence and trainingLiaise with the family, Case Manager and therapy teamHelp organise appointments, equipment maintenance and assist our clients family with any administrative tasks relating to our client.As Support WorkerSupport with all personal care, preparing meals and drinks, moving and handling and medication, whilst ensuring our clients safety and wellbeingImplement therapy programmes (physiotherapy, OT, SLT, hydrotherapy)Help plan engaging, purposeful daily activitiesCreate a positive, motivating, inclusive environmentHours & Shifts36.75 hours per week Care shifts (24.5 hours per week) and Team Leader admin duties (12.25 hours per week) (please note the administrative hours will be reviewed where this is a new position)Mix of day shifts (7:45am8pm) and waking nights (7:45pm8am)Weekend work includedHours discussed at interviewIf youre ready to bring your energy, creativity and care skills to a role where youll be truly valued, wed like to hear from you.
