Qualified Social Worker - Hospital Discharge
South Wales
All Wales Pledgerate
Temporary, 37 hours per week

About the role:
We are looking for a dedicated and passionate Qualified Social Worker to assist with Hospital Discharges in a local authority in South Wales, on a inital 3 month contract.

As a Hospital Discharge Social Worker, you will play a crucial role in ensuring the smooth and compassionate transition of patients from our hospital to their homes. You will work closely with patients, their families, and our multidisciplinary healthcare team to provide comprehensive care and support during this critical phase of their healthcare journey.

Key Responsibilities:
1. Conduct thorough assessments to identify patient and family needs
2. Develop personalised discharge plans that address patients’ unique circumstances
3. Collaborate with medical professionals, nursing staff, and community resources to coordinate care
4. Provide emotional support and counseling to patients and their families
5. Assist in the arrangement of post-discharge services and follow-up care
6. Advocate for patients’ rights and access to appropriate services

What you need:
7. A degree in Social Work or equivalent
8. Registered with Social Care WalesCompleted relevant post-qualifying training
9. Enhanced DBS check
10. Excellent communication and written skills
