About the Role
You will be working as part of a multi-disciplinary team, assessing and facilitating safe and timely discharges with a commitment to promoting independence.
Responsibilities
* Rapid Assessment under Discharge to Assess guidelines for hospital discharge
* Assessing service users, mental capacity and making best interest decisions
* Liaising with the discharge liaison team, as well as ward staff and family members.
* Arranging care packages on discharge and step down beds.
* Completing reviews and support plans.
* Reporting any delays with service users.
