Job overview

An exciting opportunity has arisen for a Hospital at Home Therapy Instructor (Band 4) to join the Brent Unplanned Care Team, based in Wembley. This role is key to supporting patients to receive high quality, person centred care within their own homes, helping to avoid unnecessary hospital admissions and enabling timely discharge from acute settings.

The post holder will play an integral role in delivering rehabilitation programmes to patients within the community and Hospital at Home service. Working under the supervision of registered therapists, including Physiotherapists and Occupational Therapists, you will support the assessment, treatment, and ongoing rehabilitation of patients with a wide range of physical and functional needs.

You will be responsible for implementing therapy treatment plans, supporting individuals to regain independence with activities of daily living, and assisting with mobility and exercise programmes. The role also involves contributing to admission avoidance and facilitating safe and effective discharge planning, ensuring patients receive the right care, in the right place, at the right time.

Main duties of the job

* Deliver therapy programmes, as directed by Physiotherapists and Occupational Therapists, including exercises, mobility practice, personal care, and non pharmacological support.
* Work with patients in community settings, including their own homes, both independently and alongside registered therapists.
* Support patients to achieve rehabilitation goals, promoting independence with activities of daily living and taking a holistic approach to care.
* Monitor patient progress and report any changes or concerns promptly to the multidisciplinary team.
* Contribute to multidisciplinary team discussions and provide updates on patient care.
* Maintain accurate and timely clinical records in line with organisational policies.
* Report incidents, risks, or concerns in accordance with Trust procedures.

Please note: You must hold a full UK driving licence and be a car driver, as you will be expected to undertake home visits.

Also this role is not eligible for sponsorship under the Skilled Worker route.

Person specification


Education/Qualification

* Broad general education including English and maths to GCSE level or equivalent
* NVQ 3 in Health or Social Care or equivalent academic qualification and /or equivalent level of experience of working as a Physio / OT / Rehab assistant


Experience

* Recent experience of working in a relevant health or social care environment
* Experience of primary care working
* Experience of working as a physio / OT / Rehab assistant
* Experience of supervising / teaching others


Skills & Knowledge

* Understanding of the skills required for effective team working
* Understanding of rehabilitation and issues relating to promoting independence
* Awareness of the impact of ageing process
* Awareness of health and safety issues related to primary care working in particular
* Understanding of supervision
* Understanding of confidentiality
* Basic numeracy skills
* Understanding of the role of a Therapy Support technical instructor including the limitations of the role
* Experience of working with ADL equipment


Other

* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
