Job Description
Trident Healthcare Solutions is seeking an experienced Band 6 Physiotherapist to join an Intermediate Care service based in Hull. The successful candidate will support discharge pathways (including DT2A) and manage a varied caseload across multiple community sites and patient homes. A car is essential for this role.
Responsibilities
* Provide high‑quality physiotherapy within Intermediate Care settings
* Support safe and effective discharge planning, including DT2A pathways
* Manage a caseload across community sites and patient homes
* Complete timely and accurate clinical documentation
* Work collaboratively within a multidisciplinary team
* Utilise SystmOne (desirable) for clinical record‑keeping
Skills and Experience Required
* Degree‑qualified Physiotherapist
* Experience within Intermediate Care, community rehab, or discharge services
* Strong clinical reasoning and caseload management skills
* Excellent communication skills
Essential Requirements
* Previous NHS experience
* HCPC xlqdzyr registration
* Car driver with access to own vehicle
* Experience in discharge pathways / DT2A preferred
