Stafford Town Primary Care Network (STPCN) islooking for an enthusiastic Clinical Pharmacist to join our rapidly expandingteam of Healthcare Professionals. The post holder is a clinical pharmacist, whoacts within their professional boundaries, supporting and working as part of amulti-disciplinary team within general practice. This post will have a patientfacing role and the post holder should be a qualified prescriber or workingtowards this. The clinical pharmacist will take responsibility for areas ofchronic disease management within the practice and undertake clinicalmedication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary supportto general practice staff with regards to prescription and medication queries.They will help support the repeat prescription system, deal with acuteprescription requests, and medicines reconciliation on transfer of care and systemsfor safer prescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patient in the GP Practice (s).
The post holder will provide clinicalleadership on medicines optimisation and quality improvement and manage someaspects of the quality and outcomes framework and enhanced services. The roleis pivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service within generalpractice.

We have a close and expanding team of clinicians serving around 44,000 patients set in Historic County Town of Stafford.
Within the Stafford Town PCN we have 4 surgeries, Wolverhampton Road Surgery, Holmcroft Surgery, Castlefields Surgery and Stafford Health & Wellbeing Surgery.

Answers relevant medicine-related enquiries from GPs,other practice staff, other healthcare teams (e.g. community pharmacy) andpatients with queries about medicines, suggesting and recommending solutions.
Providing follow up for patients to monitor the effect of anychanges
Unplanned HospitalAdmissions
Review the use of medicines most commonly associated with unplannedhospital admissions and re-admissions through audit and individual patientreviews.
Put in place changes to reduce the prescribing of thesemedicines to highrisk patient groups.
Management of Medicines atDischarge from Hospital
To reconcile medicines following discharge from hospitals,intermediate care and into care homes, including identifying and rectifyingunexplained changes and working with patients and community pharmacists toensure patients receive the medicines they need post discharge.
Set up and manage systems to ensure continuity of medicinessupply to highrisk groups of patients (e.g. those with medicine complianceaids or those in care homes).
Signposting
Ensure that patients are referred to the appropriate healthcareprofessional for the appropriate level of care within an appropriate period oftime e.g. pathology results, common/minor ailments, acute conditions, long termcondition reviews etc.
Repeat Prescribing
Produce and implement a practice repeat prescribing policy.
Manage the repeat prescribing reauthorisation process by reviewingpatient requests for repeat prescriptions and reviewing medicines reaching reviewdates and flagging up those needing a review.
Ensure patients have appropriate monitoring tests in place whenrequired.
Identification of cohorts of patients at high risk of harm frommedicines through pre-prepared practice computer searches. This might include risksthat are patient related, medicine related, or both.
Service Development
Contribute pharmaceutical advice for the development andimplementation of new services that have medicinal components (e.g. advice on treatmentpathways and patient information leaflets).
Information Management
Analyse, interpret and present medicines data to highlightissues and risks to support decision making.
Undertake clinical audits of prescribing in areas directed by theGPs, feedback the results and implement changes in conjunction with thepractice team.
Implement changes to medicines that result from MHRA alerts, productwithdrawal and other local and national guidance.
Education and Training
Provide education and training to primary healthcare team ontherapeutics and medicines optimisation.
Implementation of Localand National Guidelines and Formulary Recommendations
Monitor practice prescribing against the local health economysRAG list and make recommendations to GPs for medicines that should beprescribed by hospital doctors (red drugs) or subject to shared care (amberdrugs).
Assist practices in seeing and maintaining a practice formularythat is hosted on the practices computer system.
Auditing practices compliance against NICE technologyassessment guidance
Provide newsletters or bulletins on important prescribingmessages.
CareQuality Commission
Work with the general practice team to ensure the practice iscompliant with CQC standards where medicines are involved.

To support public health campaigns. To provide specialistknowledge on all public health programmes available to the general public.

Qualifications
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
