Please be aware this is a Long Term Temporary post to start ASAP.
The role involves attending ward reviews as part of mental health multidisciplinary teams in adults and older persons wards. Responsibilities include completing assessments of needs under the Care Act, developing strengths-based support plans, and playing a key role in discharge planning. The post holder will conduct Mental Capacity Assessments, make Best Interest decisions, and work in partnership with health, housing, and children’s services. Strong communication skills and a solid understanding of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983 are essential. The role also involves completing section 117 discharge planning meetings, Social Circumstance Reports for mental health tribunals, and conducting section 42 safeguarding enquiries when required. The team covers Merseycare wards across the region, currently based in Broadoak Unit, Liverpool.
Essential criteria:
* Degree in Social Work
* SWE Registration
* Right to work within the UK
