Neurological support worker
Dorset HealthCare is looking for a neurological support worker to join our supportive trust. You will be joining the community Neurology service that is supportive and dynamic, and will be responsible for supporting patients and the service across East Dorset. We welcome ideas of change to create a better service for people living with long term neurological conditions.
We embrace flexible working and support a hybrid model of working from home as well as the office. Hours can be flexed over working days to meet service needs.

Responsibilities
* Work within the community neurology service to support clients who have a long term neurological condition living within the community.
* Assist during joint assessments alongside professionals and maintain independent contact with clients and/or carers once care has been prescribed.
* Support, monitor and provide interventions to clients and their carers in their own home and/or local community.
* Be supported by neurological specialist therapists including occupational therapists, physiotherapists and a team lead; participate in CPD opportunities and supervision.
* Contribute to service improvement by sharing ideas for change to benefit people living with long term neurological conditions.
Qualifications and Requirements
* Enhanced Disclosure and Barring Service (DBS) check required; the Trust will cover the cost. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of a new check.
* Applicants requiring current Skilled Worker sponsorship to work in the UK are ineligible to apply for this position.
* Flexibility in how hours are met over the working days, with a hybrid model of working from home and office.
