This role will primarily support the youth justice service with health assessments and identifying health needs.
Your responsibilities will encompass assessing, planning, delivering, and evaluating holistic health interventions for children and young people facing a range of challenges, including:
* Navigating the complexities of Child Protection plans.
* Supporting Children In Need with compassion and care.
* Advocating for the wellbeing of Children Looked After, ensuring their voices are heard and needs are met.
* Empowering young parents grappling with intricate health and social needs.
* Addressing the concerns of children missing education and those in alternative provision.
* Facilitating coordinated interventions for vulnerable children with multifaceted lives.

Main duties of the job
As a Child & Family Targeted Nurse (CFTN), you'll be at the heart of safeguarding and supporting families in need, working collaboratively with a variety of agencies to ensure children's safety and well‑being.
In this vital role, you'll take the lead in championing the development and implementation of safeguarding policies and procedures. You'll support the delivery of clinical and safeguarding supervision for the team, ensuring the highest standards of care for every child.
Partnering with Specialist Public Health Nurses (Health Visitors and School Nurses), you'll help deliver the Healthy Child Programme, providing exceptional care to children and young people from birth to 19 years. Working within a dynamic, multidisciplinary team, you'll be part of a community dedicated to improving the health and future of Oldham's children.
This role offers flexibility, as you'll be based for administrative purposes at one location, but may be asked to work from various sites across Oldham, depending on the needs of the service. If you're ready to be a vital part of our integrated Children & Families Service, we want to hear from you.
Qualifications
* Registered Nurse Adult or Registered Nurse Child Branch / Registered Midwife
* Degree in Nursing / Health related or currently studying towards or equivalent experience
* Non-medical prescribing V100/150 or willingness to complete
* Sexual Health screening training /Qualification E.g. CRASH level 1
* Specialist Community Public Health Nursing Health Visiting or School Nursing
* Evidence of further qualification or working towards relevant master's module(s) or equivalent level of training in respect of child health and development e.g. safeguarding/CBT/perinatal mental health
* Assessor and supervisor qualification
Professional Registration
* Registered Nurse with current NMC registration
Knowledge, Training and Experience
* Demonstrable extensive post qualification experience of working with children/young people in a community setting in the NHS with significant experience working with vulnerable children, young people and families
* Working with children/young people with behaviour that challenges
* Working with adolescents
* Working within a multi-disciplinary and multiagency team
* Identifying and working with Safeguarding Children Issues
* Participating in Safeguarding supervision
* Assessment and report writing
* Extensive Knowledge of Child Protection/Safeguarding issues, policies and procedures
* Up to date knowledge of the literature relating to the needs of the child & young People and to the agencies supporting them
* Awareness of factors that contribute to good health and the importance of promoting these in line with public health policy and practice
* Developing and facilitating training and/or health promotion sessions
* Understanding the application of the Mental Capacity Act 2015
Skills and Abilities
* Excellent written, verbal and interpersonal communication skills
* Ability to adapt communication styles to match patients, carers, colleagues and partner's needs
* Ability to assess health needs and plan, develop, implement and assess programmes of care for children/ young people
* Ability to communicate complex information and advice on healthcare to clients, carers and colleagues
* Very high-quality written communication skills adaptable for a wide and varying readership
* Ability to use IT skills for report writing use of databases and communication
* An ability to develop effective working relationships across agencies
* Adaptability to working in a wide variety of settings
* Must be able to transport self around the community
* Ability to organise workload with minimal supervision
* Ability to travel to different geographical locations across the Trust footprint in line with service needs
* Full UK driving license with willingness to use own car for work purposes
