Location: Middlesbrough
Type: Permanent, Full Time (37.5 hours per week)
Salary: £25,838 per annum plus benefits including Health Cash Plan
About the Role
Are you passionate about supporting individuals with complex housing and support needs? Join us as a Support Coordinator and be at the heart of empowering our customers to live more independently within the community. Your daily efforts will help customers achieve their goals and improve their quality of life.
Responsibilities
* Develop and review support plans with customers
* Support customers with daily activities such as cooking, shopping, budgeting, and maintaining their tenancy
* Assess risks, set goals, and conduct regular reviews
* Build strong relationships through regular catch-ups and support sessions
Candidate Profile
* Experience in creating person-centred support plans
* Ability to coordinate customer referrals and work independently
* Resilient, calm under pressure, and detail-oriented
* Must have a vehicle insured for business use; mileage paid by us
Additional Information
Due to sponsorship restrictions, we cannot currently offer sponsorship to external candidates. The role requires working on a rota basis, including weekends, with opportunities for flexible start and finish times. Regular team meetings and training sessions are part of the schedule. An Enhanced DBS check is required, which we will cover.
