The Specialist Community & Crisis Support serviceprovides 24-hour specialist advice, support and facilitation of care for thosepersons with complex symptom management due life limiting illness and supportto carers of the individual within the home setting, at a nurse led clinicenvironment or within the office on telephone support.

Our Referral Hub receives all referrals for individualsbeing referred to the hospice services, assessing and identifying thespecialist interventions required

An opportunity has arisen for adynamic, enthusiastic, self-motivated, experienced nurse with good clinicalpalliative care knowledge base, to join our well-established specialistcommunity services as a Clinical Nurse Specialist (Bank). This post willprovide varied senior experience in a multidisciplinary working environmentdelivering a 24/7 service to individuals, carers and other health careprofessionals in the community.

We are looking for a candidate whois working at a senior lever and hasexcellent communication, assessment and prioritisation skills, and understandsthe needs of those receiving palliative care /end of life care, a workingknowledge of evidence -based practice and the ability to develop a goodknowledge base of current initiatives and developments in palliative care.

The Service works within theboundaries of Barking & Dagenham, Havering, and Brentwood and provides outof hours advisory telephone support to Redbridge.


Main duties of the job

Fixed Bank Salary of £47,665 per annum, pro rata (£24.38 per hour)

To workcollaboratively with the palliative care team and primary care teams providinghigh quality specialist support, advice and symptom control to Individuals withspecialist complex palliative care needs and their carers in their primary caresetting.

To providespecialist telephone assessment and support for healthcare professionals,Individuals and relatives as required via the Specialist Advice Line, whilstdocumenting with accuracy.

To work bothautonomously and a team member to provide a specialist resource in palliativecare and work in close collaboration with acute hospital and other specialistpalliative care services and the wider multidisciplinary team.

To providespecialist support, decision-making skills, expert advice/consultancy andeducation to members of the primary care team to support them with theirmanagement and care of people with life limiting illness in thecommunity.

Toprovide rapid crisis access to specialist advice for professionals, persons inthe community and those involved in their care to crisis telephone support,assessment, decision making, prioritisation of need and face to face crisisvisiting as appropriate.

We support 5 Clinical Commissioning Groups & have a robust governance structure in place. We provide ward clinical services within a modern, personal & friendly 18-bedded unit, as well as Specialist Community Crisis Support Service, Hospice at Home Service & Specialist Day Therapy Service. We also have the hospice Pepperell Education Centre that has an excellent reputation for delivering high quality education programmes and study days.

Job responsibilities

MAINDUTIES AND RESPONSIBILITIES

CLINICALEXPERTISE, LEADERSHIP AND TEAMWORKING

Workwithin community services to respond to referrals and Individuals on thetelephone to offer skills of a clinical nursespecialist.

Providespecialist holistic telephone assessment and support ongoing for healthcareprofessionals, Individuals and relatives in line with service standards.

Accuraterecording of interactions and information gained for an Individual.

Towork within an allocated Clinical Nurse Specialist role daily to ensure smoothservice provision and allow for flexibility of service demand.

Tobe part of the team, carrying out a holistic telephone assessment and completeall relevant documentation, liaise appropriately with primary health careteam to resolve problems identified provide them with a contact details of theservice.

Provideexpertise in complex symptom control and advise any health care professionalsassociated with palliative care, helping them to evaluate the effects ofprescribed interventions and recognise and appropriately manage palliative careemergencies.

Offerskilled psycho/social support for Individuals with specialist palliative careneeds and their families, providing them with appropriate information abouttheir condition, its management and the services available to support them bothpre and post bereavement.

Takeresponsibility for assessment and clinical judgement of those persons at homeknown to the service for referral request for admission to the hospiceward.

Provideongoing telephone support to Individuals / families / carers whilst theIndividuals is waiting to be seen by other hospice services.

Gather,document and co-ordinate information from other professionals and thespecialist care teams relating to Individuals referred to communityservices.

Fullyunderstand the importance of multi-professional team-working and the ability tochampion it within the organisation.

Havean understanding and awareness of behaviours and interpersonal skills that arevital to effective team working and practice them at all times and ongoingcommitment to service and organisation values.

Possessa proactive and supportive leadership style that encourages team participationin decision-making processes.

Maintainaccurate, up to date documentation in accordance with Saint Francis Hospicepolicies and within guidelines laid down by the NMC.

Input,record and maintain all form of records (written and electronic), data andstatistics onto I-Care and any other data collection systems in place inaccordance with hospice policies.

EDUCATIONAND QUALITY

Attendmandatory training as specified by the hospice.

Takeresponsibility for own professional development, in accordance with appraisaland performance review. Attend study courses in agreement with line manager andmaintain current registration of the required qualifications for the post withthe NMC.

Ensurenursing practice in the field of palliative care remains evidence based andensure active dissemination of relevant findings.

Participatein research programmes as appropriate using the findings to enhance clinicalstandards, clinical effectiveness and outcomes.

Ensurethat post registration training meets the requirements necessary to beconsidered a clinical nurse specialist within the field of palliative carenursing.

Generateand use appropriate learning opportunities and apply own learning to the futuredevelopment of practice-attendance at organisational Specialist Updatesencouraged.

Articulateand reflect palliative care skills, enabling others to learn and be responsiblefor acting as a resource and sharing of knowledge to help others developincreased skills across the organisation.

MANAGEMENT

Establishand maintain effective lines of communication with hospital, multi services inthe community and other organisations to ensure continuity of care.

Participatein forums for discussion on research and audit programmes, and in clinicalgovernance initiatives e.g. clinical review.

Participate& engage in group clinical supervision and self-evaluation and use this toimprove care and practice.

Responsiblefor management and organisation and prioritisation of work within core hours onduty as part of organisational service provision.

PROFESSIONALRESPONSIBILITIES

Toact within the legal and statutory rules relating to nursing practice as laiddown by the Nursing & Midwifery Council.

Tobe responsible and ensure adherence to professional Revalidation expectationsand process.

Toadhere to the policies, procedures, standards and conditions of service ofSaint Francis Hospice relating to sickness and absence, conduct, Health andSafety, Data Protection, and any others that are relevant.

Totake responsibility for own professional and personal development which will bemonitored in supervision sessions and in the annual appraisal.

To keep up-to-date with the developments within nursing and in particular inthe field of palliative care.


Person Specification


Skills/Abilities/Knowledge/Communication

* Excellent symptom control knowledge.
* Knowledge of current issues in palliative care.
* Evidence of recent and ongoing professional development in palliative care.
* Ability to work autonomously as well as part of a team both within hospice MDT, primary health care services and hospital trust teams.
* Motivated and enthusiastic with ability to be flexible, innovative and keen for ongoing service development.
* Advanced communication skills and evidence of collaborative working.
* Ability to manage own work and work within a corporate caseload.
* Good IT skills and knowledge of electronic healthcare systems.
* Good personal support mechanisms.
* Community based experience.
* Flexible approach to team working and an awareness of effective team dynamics.


Qualifications

* RGN with significant post registration experience.
* Oncology /Palliative Care Qualification ENB 237/285 or equivalent.
* Teaching Qualification or equivalent level of experience.
* Degree or equivalent level of experience.
* Significant senior post registration experience at a similar level in palliative care or another associated speciality.
* Counselling qualification/experience.
* Non-Medical prescriber or working towards.
