As a Social Worker in the Care and Support Team you will work with people over 18 years of age with a mental illness or physical disability. You will work closely with the appropriate primary care teams fostering collaborative relationships which lead to better outcomes for the people we support. What we are looking for: Enthusiastic, qualified and experienced Social Workers with a commitment to continuous development.
Responsibilities
In this role, you will conduct comprehensive assessments and enjoy a healthy balance of autonomy and support. You’ll manage your own caseload while receiving regular supervision and consistent guidance from Advanced Social Work Practitioners and your Team Manager. A clear understanding of the Care Act and the Local Authority's statutory duties is essential. Requirements
* Degree in Social Work & registered with Social Work England
* Successfully completed the UK Skills for Care ASYE programme
* A valid UK driving licence and is either a car owner or can use a car for work purposes.
