Job Description:
Within the framework of Community Care legislation and professional guidance, undertake outcome-focused assessments and care management tasks for older people and their carers. This work will be supported by regular supervision and advice. It does not involve decisions related to legal powers or duties that could lead to deprivation of liberty. Ensure risks to service users and carers are identified and managed within policy guidelines.
Design tailored service packages to meet assessed needs, including aids to daily living, aligning with departmental policies and priorities, and agreeing objectives with individuals.
Negotiate effective ways to achieve these objectives and liaise with other professionals and agencies involved in care provision, incorporating them into individual care plans. Ensure service users and carers receive copies of care plans and that actions are properly recorded.
Maintain close contact and coordinate with other department staff and agencies such as Housing, Health, and voluntary or independent sector organizations to ensure integrated service delivery.
Oversee monitoring, reviewing, and reassessing care packages, and escalate concerns about service provision to facilitate appropriate action. Contribute to multidisciplinary reviews and prepare necessary reports. Responsible for approving care plan variations within set parameters.
Be aware of the diverse ethnic and cultural needs of the local population. Develop and deliver culturally sensitive care packages, seeking advice as needed.
