We are seeking a highly organised, compassionate, and experienced healthcare professional to join our team in a dual-role position combining pre-admission assessment and Continuing Healthcare (CHC) coordination.
This role is central to ensuring that prospective residents receive timely, appropriate, and safe placements, while also ensuring robust coordination of CHC and Funded Nursing Care (FNC) processes. The post holder will play a key role in assessment, funding advocacy, documentation, and liaison with NHS and local authority partners, supporting both high-quality care planning and accurate funding recovery.
Key Responsibilities
Pre-Admission Assessments
* Conduct comprehensive pre-admission assessments to evaluate care needs, risks, and suitability for placement.
* Maintain awareness of bed availability across homes and liaise with Home Managers regarding room allocation and equipment requirements.
* Coordinate assessment arrangements with referring agents, including families, social workers, hospital discharge teams, and health professionals.
* Admission Coordination
* Create suspended initial care files on PCS, including diet, allergies, equipment needs, and identified risks.
* Request and obtain proof of LPA prior to admission where applicable.
* Communicate admission details clearly to Home Managers, Head Office finance, and home-based administrative teams.
DST Meetings and CHC Coordination
* Coordinate, prepare for, and attend Decision Support Tool (DST) meetings.
* Compile and submit accurate documentation to support CHC eligibility assessments.
* Liaise effectively with NHS CHC teams, commissioners, and multidisciplinary professionals.
Funded Nursing Care (FNC) Monitoring
* Monitor FNC eligibility and payments for qualifying residents.
Communication Lead
Responsibilities will include:
* The postholder will take a proactive role in supporting and enhancing communication across the organisation.
* Undertaking basic website maintenance, including updating content, ensuring information is accurate and current, uploading news items, events, and relevant documentation.
* Working collaboratively with each of the three Homes to plan, coordinate and produce regular newsletters for families of residents
Qualifications
* Registered Nurse (RGN/RMN) or equivalent qualification (essential).
* Experience in care homes, hospital discharge, or community nursing (desirable).
* Sound knowledge of Continuing Healthcare (CHC) and Funded Nursing Care (FNC) frameworks.
Skills and Experience
* Strong clinical assessment and care planning skills.
* Excellent written and verbal communication skills.
* High level of organisation and ability to manage multiple priorities.
* Experience working with NHS systems and funding pathways.
Hours: Part Time – 30 hours per week. Whilst the majority of the working week will be during office hours, there may be a requirement to work outside of normal office hours to meet the demands of the role.
Location: Hybrid – You will work across our 3 Homes in Keswick, Kendal and Carlisle. You will be expected to carry out pre-assessments across the County and occasionally further afield.
