District Nurse / Case Manager, SMASH
An exciting opportunity for a Band 6 District Nurse / Case Manager in Alsager within the SMASH Care Community Team as a secondment post for 7 months.

Main duties of the job
In the role as a District Nurse / Case Manager you will work in partnership with patients and their families to lead in the delivery of person‑centred, safe and effective care. You will demonstrate sound clinical knowledge and skills in managing patients with complex needs, long‑term conditions and end‑of‑life care.
Requirements include extensive clinical experience and case‑management experience, preferably within the community setting, with evidence of recent and ongoing clinical professional development relevant to this post. You will be supported to develop leadership, organisational and communication skills and to manage challenging situations professionally and flexibly.
Although based in Alsager, there will be occasions requiring work across the Central Cheshire Integrated Care Partnership footprint. Travel is essential for this role; you must demonstrate how you will meet this requirement.
Job responsibilities
The post holder will manage and coordinate patients requiring community nursing input, overseeing assessment of complex nursing care needs, and developing, implementing and evaluating personalised care programmes for patients on a defined district nurse caseload.
They will deputise as required in the absence of the team leader to ensure efficient and effective service delivery, and work as part of the integrated community health care team across professional and organisational boundaries covering 24‑hour service delivery.
They will assist and support the district nurse team leader in providing clinical leadership to the community nursing team and assist in leading the team in delivering high‑quality, evidence‑based care programmes.

* Excellent interpersonal and communication skills, written and verbal
