Adult Social Care currently has a Social Worker vacancy to join our Mental Health Hospital Discharge Countywide team, on a fixed-term contract for 12 months. You will work with working-age adults (18-70) and older adults (70+), supported by a team that cares for you as much as it cares for our community.

Main duties of the job
We are looking for a Social Worker to support effective and timely hospital discharges. You will work across East Sussex covering all acute hospitals for people admitted for mental health treatment. This is a new enterprise; you will work as part of a remote partnership team with Sussex Partnership Foundation Trust (SPFT) and the voluntary sector to identify safe, creative discharges, placing the adult at the centre.
You will collaborate closely with other stakeholders to achieve optimal discharge plans.

Job responsibilities
1. Undertake tasks such as safeguarding investigations, needs assessments, risk management, commissioning support, and reviews.
2. Identify and report safeguarding concerns, liaising with agencies, and acting as Investigating Officer as delegated.
3. Support outcome-based planning for adults using legal interventions and policies.
4. Work directly with adults, carers, families, and communities to support informed decision-making and participation.
5. Enable adults and families to build community support networks and maintain independence.
6. Maintain case records and present reports to panels and courts as appropriate.
7. Contribute to the learning of students and team members through supervision and mentoring.
8. Engage in continuous professional development aligned with the Professional Capability Framework.
Mental Health addendum
In addition to the above, Mental Health Social Workers will:
9. Train as an Approved Mental Health Professional (AMHP) within two years post-ASYE.
10. Act as an AMHP and contribute to rotas, possibly working after hours.
11. Provide practice placements for AMHP students.
Person Specification
Qualifications
* Social work qualification and registration with the regulatory body
* Completion of ASYE or equivalent
* Relevant postgraduate learning
* Level 1 Practice Educator Standards or equivalent
