Thepost holder will be working within a multidisciplinary team of healthcareprofessionals to provide a high-quality service to the patients of Tower FamilyHealthcare by working closely with our established Practice Pharmacy team andPrescriptions team, to support the development of high quality, safe andcost-effective prescribing in the practice, utilising and reviewing prescribingsystems, implementing processes to ensure evidence-based practice is embeddedin patient care, identifying and streamlining medication review process,assisting with improving medicines optimisation and carrying out medicationaudits.
Main duties of the job
Contributingto the provision of safe and effective pharmacy services
Undertakingmedicines reconciliation in line with practice protocols and standard operatingprocedures to improve the efficiency and effectiveness of prescriptions andrepeat prescriptions
Participatingin medication audits
Providingadvice and information to all staff and patients to support the improvement ofprescribing safety, quality, cost effectiveness and the appropriate use ofmedicines to get the best outcomes for patients
Assistingin the monitoring and management of prescribing budgets and the preparation ofevidence-based resources and information that support cost-effectiveprescribing
Monitorlocal and national prescribing targets, reviewingprescribing patterns, financial changes, and adherence to local guidelines andformularies
Takingresponsibility for contributing to all aspects of the day-to-day management ofthe working environment including communication, data accuracy and management,keeping practice areas tidy and helping to achieve a supportive, positiveculture across the practice team
Tosupport and promote a safe working environment in accordance with Health &Safety and Infection Control policies
To maintain clear,accurate and contemporaneous computerised records of all patient contacts and respect and protect patient confidentiality andinformation
To assist thepractice in managing the introduction of new drugs and drugs that have beenwithdrawn

Our clinical team is diverse including GPs, a Practice Paramedic, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Practice Nurses, Nurse Associates, a Physician Associate and highly skilled Health Care Support Workers, working closely with a range of additional roles including an Acute Visiting Service, Mental Health Practitioners and First Contact Physiotherapists.

Communication
To work closely andcollaboratively with other members of the practice management team to achievehigh quality, safe and evidence-based patient care consistently across allsites of Tower Family Healthcare
To work collaborativelywith both clinical and non-clinical staff to deliver seamless care to thepatients of Tower Family Healthcare.
To workcollaboratively with the ICBs Medicines Optimisation Team to support theimplementation of medicine related ICB initiatives within the practice.
To liaise closelywith local community pharmacists, community nurses, care home staff, hospitalpharmacists and other care providers, as necessary to ensure accurate exchangeof information/clarification of queries regarding medicines.
To communicateeffectively to a variety of healthcare professional, patient, and communitygroups. This may involve small group sessions, public speaking or makingpresentations as appropriate.

Person Specification
Qualifications
* Registered with the GPhC as a Pharmacy Technician
* NVQ Level 3 Pharmacy Services or equivalent
* Additional training courses relating to pharmacy studies
