Job Overview
This is a post in primary care within the community mental health team in Conwy.
We provide assessments from numerous agencies who refer through our SPOAA daily meetings, and offer brief interventions within groups and occasionally on a one-to-one basis. A good knowledge of third sector agencies is useful as we work hard to become more familiar and signpost to these very helpful agencies, continually improving our contact and working relationships.
