CATAT - Community and Acute Tiered Assessment Team
After our first year of operation in 2023, CATAT was highly recommended for the Working Together for Better Mental Health award at the staff excellence awards. We are seeking professionals with a keen interest in working with children, young people (CYP), and their families to help them achieve better outcomes and support community-based care.
The aim of our team is to prevent CYP from requiring inpatient admissions whenever possible, by supporting them and their families proactively within the community. The team acts as a gateway in and out of Tier 4 (T4) inpatient services. CATAT collaborates with core community teams and partner agencies to provide CYP with increased and additional support, fostering joint risk management, collaborative working, and continuity of care.
Our Main Pathways
* Admission Avoidance: The least restrictive option, supporting CYP already engaged with a core team who need more intensive intervention to prevent worsening conditions or hospital admission.
* In-reach/Outreach: Support for young people transitioning from inpatient settings (T4) back into the community, helping them adjust and reintegrate.
Between 07/2022 and 08/2024, our dedicated team has successfully avoided 165 inpatient admissions. If you are a dynamic individual passionate about making a difference in services supporting young people and families, we invite you to join us and help continue our success.

* A systemic approach involving CYP, families, carers, and wider professional networks
* Training and education for other services
* Promoting joined-up working and liaison across mental health, physical health, and children’s services
