Our client is seeking a skilled and dynamic Occupational Therapist to join their Reablement and Hospital Discharge pathway in Wiltshire. The role has arisen due to an increased workload linked to the successful implementation of the Home First contract. The postholder will support discharges from GWH and RUH acute hospitals and provide community-based interventions aimed at enabling individuals to return home safely and independently. Key Responsibilities Work collaboratively with acute hospital teams at GWH and RUH Support timely, safe hospital discharge planning Conduct home and community visits, assessing functional needs Develop goal‑oriented, person‑centred reablement plans Provide evidence-based interventions to promote independence Maintain accurate, timely documentation Contribute to MDT discussions supporting effective discharge pathways Working Model & Expectations Admin Time: 50% of admin must be completed in the office Remaining admin can be completed from home Visits: Community visits as needed across the patch Example schedule: 3 days of visits → 1 day office admin → 1 day WFH Important: Wiltshire Council does not pay accommodation or travel expenses for candidates living outside Wiltshire Essential Requirements HCPC‑registered Occupational Therapist Experience in: Acute hospital discharge work Community OT assessments Reablement / Home First / intermediate care High-paced clinical environments Strong clinical judgement and autonomy Full UK driving licence and access to a vehicle (preferred) Why Liquid Personnel?
