Aberdeenshire Discharge Coordinator - ARI Band 6 - 30 Hours per week - Permanent Aberdeenshire Discharge Team is looking for a hardworking, dynamic, proactive, and friendly nurse to join their liaison team in the Band 6 role. Aberdeenshire Discharge Hub facilitates the safe and effective discharge of Aberdeenshire patients from Aberdeen Royal Infirmary to home, community hospital or other care setting in a timely fashion ensuring appropriate length of stay in the acute hospital setting. The post holder will work with Multidisciplinary Teams and Care management Team colleagues in determining discharge needs of patients and their families/carers thereby proactively moving the patient on to the most suitable place of care by using specialist knowledge, networking skills and effective communication, to educate, provide advice, support and expertise to staff to facilitate efficient movement of appropriate patients. The post holder will be responsible in supporting complex discharges to long term care facilities and support communication between care management and prospective care homes. The discharge hub team work a 7 day a week service between the hours of 0800hrs to 1600hrs. You will demonstrate excellent leadership, confident communication, and effective organisational skills. Evidence of personal development will be expected. You should possess effective communication skills and have the ability to work as an individual and as part of a team. We are looking for an individual with enthusiasm and commitment who will enjoy the challenge the role offers. General medical or community nursing experience in NHSG is essential.
