Pro rata (dependent on experience & qualifications)

Permanent subject to funding


KEY TERMS & CONDITIONS:

* 29.6 hours per week, 25 days holiday pro rata, 1-day birthday leave, 8% non-contributory pension.
* Cycle to Work scheme – Buy a bike tax free and cycle to work.
* Westfield Foresight Health Plan – Annual benefits and services such as routine dental work and check-ups, etc.


Role Overview:

The Family Welfare Coach supports families facing complex challenges by building strong, trusting relationships and walking alongside them to improve wellbeing, confidence, and independence. Using a trauma-informed and culturally responsive approach, you will offer holistic coaching support, help families navigate services, and connect them to social prescribing pathways and community-led solutions. The role will:

* Coordinate, simplify and broker access to a range of health and social interventions based on the needs and wishes of the individual, including liaising with agencies such as schools, housing, and Family Intervention Service (FIS).
* Empower individuals to express their views, wishes, and concerns.
* Co-produce a personalized support plan that encourages empowerment and self-management ('what matters to me').
* Facilitate delivery of a peer-led ‘Mindful Mamas’ group.
* Connect individuals to local and city-wide services.
* Develop close working partnerships with health, social care, housing, and voluntary, community or social enterprise (VCSE) organizations.


Additional Requirements:

An Enhanced DBS check is required for this role.
