Purpose of the Role
Social prescribing empowers people to take control of their health and wellbeing. As a Social Prescribing Link Worker (SPLW), you will take a person‑centred, holistic approach to connect individuals to community groups and statutory services, offering practical and emotional support that helps them live healthier, more fulfilling lives.
Key Responsibilities
* Manage a caseload of people with complex social needs, referred by GPs, Advanced Nurse Practitioners, and First Contact Practitioners.
* Conduct comprehensive assessments and create personalised care plans.
* Help individuals identify issues affecting their health and wellbeing.
* Connect people to non‑medical, community‑based activities and services (e.g., arts, physical activity, nature, specialist advice).
* Use coaching techniques to empower individuals to take control of their wellbeing.
* Build strong relationships with local services and community groups, supporting the development of new initiatives.
Knowledge & Experience
* Experience working within a multidisciplinary team.
* Background in health, social care, or community development.
* Ability to collaborate effectively across sectors.
* Understanding of social determinants of health and long‑term condition management.
Qualifications
* NVQ Level 3 in Social Prescribing (or equivalent), or willingness to work towards it.
* Training in health coaching, motivational interviewing, or behaviour change techniques.

* Excellent teamwork and ability to integrate into primary care teams.
