In this telephonic role, you will guide our members through their healthcare journeys, ensuring they receive the best possible care and support.
This position focuses on proactive engagement with high-risk and rising-risk members, especially during discharge planning and care transitions. You will collaborate with a multidisciplinary clinical team, including healthcare professionals, care coordinators, and records specialists, to deliver innovative remote care.

Schedule
* CA Based/Licensed: Monday - Friday, 9:00 AM - 6:00 PM PST
* Non CA Based/Licensed: Monday - Friday, 9:00 AM - 6:00 PM local time zone
Your Responsibilities
* Conduct high-value Care Management outreach calls to engage high-risk and rising-risk members, fostering strong relationships and promoting proactive health management.
* Make proactive calls to members post-hospital discharge, conducting thorough intake assessments.
* Collaborate with hospital-based case managers to support members' specific discharge needs and encourage engagement with our Care Management program.
* Perform initial assessments covering daily living activities, cognitive functions, social determinants of health (SDOH), and life planning to develop holistic care strategies.
* Deliver coordinated, patient-centered virtual Care Management via telephone and/or video to improve health outcomes.
* Create impactful care plans in collaboration with members and our multidisciplinary care team, empowering them to achieve their health goals.
* Coordinate necessary resources to address members’ clinical and social needs holistically.
What We’re Looking For
* Bachelor of Science in Nursing (BSN)
* Compact Nursing License with an active California Registered Nurse (RN) and Illinois License in good standing.
* 5+ years of clinical nursing experience
* 2+ years in care management, case management, or disease management, preferably in a health plan or third-party administrator (TPA) environment.
* Experience in remote care or case management
* Comfortable discussing a variety of medical conditions and working with diverse populations.
* Strong technological skills and proficiency in using multiple computer/medical record systems.
Compensation & Benefits
Hourly Rate: $33 to $40
Compensation may vary based on skills, experience, and education.
