About this job

A formal job plan will be designed and agreed with the successful candidate.

Clinical Responsibilities:

* The post-holder will be available to undertake surgery consultations, telephone consultations, visiting patients at home and in nursing homes, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
* Required to be on-call for clinically urgent and emergency problems and be included in the doctors on-call rota.
* Required to make home visits when on call or duty doctor within the session time specified.
* Make professional autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
* Screen patients for disease risk factors and early signs of illness.
* Cover all clinical management of patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or other providers as appropriate.
* Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols and wider guidance such as that from NICE.
* Ensure appropriate follow up of patients.
* Record clear and contemporaneous computerised consultation notes to agreed standards.
* Collect data for audit purposes.
* Compile and issue computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible.
* Prescribe generically whenever appropriate and in accordance with the Practice prescribing formulary, NICE guidance, Good Prescribing Practice.
* Work with and support other members of the primary health care team in delivering high quality primary care.
* Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council and Department of Health.
* Play a role in the practice achieving QIF/ Collaborative priorities and other key performance indicator targets.

Other Responsibilities within the Organisation:

* Aware of and comply with all relevant Practice policies/guidelines e.g. prescribing, confidentiality, data protection, health and safety.
* Contribution to achievement of highest possible quality standards such as QOF/Cluster priorities and other agreed Key Performance Indicator targets.
* Commitment to life-long learning and audit to ensure evidence‑based best practice.
* Contribute to evaluation/audit and clinical standard setting within the organisation.
* Attendance at regular in‑house clinical, educational meetings and training as well as events with other agencies as appropriate.
* Assistance with the training of other staff members and also with the education of doctors and nurses in training as appropriate.
* Other duties or activities as agreed by the partners.

* Assist in promoting and maintaining personal and others' health, safety and security as defined in Health & Safety Policy, including use of personal security systems within workplace, identifying risks, undertaking activities to manage risks, using training to update knowledge, infection control procedures, maintaining work areas tidy and safe, reporting potential risks.

The post holder will maintain continued education through attendance at courses and/or study days necessary to ensure professional development requirements for appraisal and revalidation are met. This may include training needed for areas of Practice Development.

* Apply Practice policies, standards and guidance.
* Discuss with other team members how policies, standards and guidelines affect own work.
* Participate in audit where appropriate.
