Community Mental Health Nurse Kent and Medway NHS and Social Care Partnership Trust
Join our team as a Community Mental Health Nurse within the Crisis Resolution Home Treatment Team, delivering support to patients and families in their homes. Utilize evidence-based, profession-specific, and general mental health knowledge to provide client-centered, recovery-oriented care for individuals, families, and carers referred to the service.
You will also work within the Rapid Response function, undertaking assessments 24/7 from the Urgent Mental Health Helpline and conducting gatekeeping assessments. The role involves working in accordance with Trust policies, Crisis Resolution Home Treatment Team procedures, and the Care Programme Approach. The aim is to offer an alternative to inpatient admission by treating patients in their home environment during crises with significant distress.
Key Responsibilities
* Work as part of a team and independently make autonomous decisions in a fast-paced environment.
* Support Band 7 colleagues in managing a caseload of service users with complex, acute, or crisis needs.
* Balance all aspects of the role, demonstrating flexibility and responsiveness to changing demands.
* Participate in planning, delivering, and evaluating services, including training and supervision of students and staff.
* Lead shifts, allocate work within the multidisciplinary team, and prioritize workloads.
* Develop and implement individual care plans and contribute to service development initiatives.
