Job responsibilities
* To work in partnership with the consultant medical staff in a specialist clinical role providing high‑quality care for patients presenting with cardiology conditions including ACS, arrhythmia, and heart failure. Care will be delivered in cardiology SDEC, clinics and ward settings, including urgent care areas and medical wards across all sites (Alexandra, Worcester, and Kidderminster).
* To act as the lead practitioner in the rapid access chest pain service.
* The post‑holder will work as an integral part of the multidisciplinary cardiology team, providing expert relevant clinical advice and support to patients and their carers within Worcestershire Acute Hospital.
* The post‑holder offers nurse‑led outpatient services and manages a caseload of inpatients on the acute site.
* The post‑holder will represent the cardiology service at Trust meetings and other meetings outside the organisation, acting as an advocate for patients within the service.
* Participate in developing service criteria, pathways of care, protocols and guidelines, as well as service development encompassing audit, research, innovation, and evaluation.
* The post‑holder will communicate at an advanced level of practice in very difficult situations with a variety of staff and agencies.
* Act as an advocate for patients always, ensuring a person‑centred approach when overseeing the patient treatment and investigation pathway, ensuring clear and concise communication to all parties.
* To act as an expert practitioner to provide direct, specialised care including interventions, procedures, and advice to patients within the cardiology service, demonstrating advanced clinical skills and knowledge in the assessment and treatment of individual patients.
* Take a professional approach to leading on flexible, innovative and creative working practices, and implement changes to enhance patient care where appropriate.
* To undertake the comprehensive assessment of patients, including those with complex and multi‑pathological cardiac conditions using highly specialised analytical and clinical reasoning assessment skills to formulate an accurate differential nursing diagnosis and prognosis.
* To demonstrate excellent ECG interpretation skills and knowledge of cardiac investigations including ETT, CTCA, echocardiogram, stress echo, and cardiac MRI.
* To apply advanced clinical knowledge and skills in the assessment and management of patients using approved medical and nursing models, taking an individualised patient approach and working collaboratively with the cardiology teams across sites.
* To initiate timely onward referral of patients as necessary.
* Responsible for escalating issues appropriately and in a timely manner when any situation is beyond the expertise of the post‑holder.
* To use advanced judgement skills and a highly comprehensive knowledge of nursing and medical interventions to devise a specialist, individualised treatment and management plan, reflecting evidence‑based practice incorporating secondary prevention.
Person Specification
Experience
* Advanced knowledge of the range of presentations, diagnoses & current management options within the specialty area.
* Evidence of planning and organisational skills.
* Independent working and autonomy.
* Previous clinical experience including setting up and running clinics or services.
Qualifications
* Registered Nurse on sub‑part 1 or 2 of the professional register.
* Cardiology care course or equivalent.
* Adult health assessment module.
* Advanced communication or working towards.
* Consent or working towards.
* Leadership course.
* Recruiting and retaining staff or equivalent.
* Portfolio of evidence in line with specialty.
Skills and Knowledge
* Advanced ECG skills.
* Advanced knowledge of the range of presentations, diagnoses & current management options within the specialty area.
* Advanced Life Support.
* Venepuncture and cannulation.
* Demonstrates good communication skills, including emotional resilience, conflict management and team working.
* Able to work autonomously within the boundaries of the role and skills.
* Ability to work flexibly within the confines of the team and service.
* Able to travel across sites.
