Job overview Applications are invited for a full time Consultant in Old Age Psychiatry based at Bushey Fields Hospital Dudley to join 2 existing Consultants.
Full time - 40 hours per week (40 hrs / 10 x PA's) This existing post is within a well-established supportive service working both on the in-patient wards and in the community, having retained the Continuity of Care model.
The Consultant will lead a team of other doctors; CPNs; Social Workers and Occupational Therapists, and together with the existing workforce cover the communities of Dudley, Stourbridge and Halesowen in the Black Country.
We are also developing new models and methods of working which will include liaison to primary care within the MCP model, and have developed an Old Age Home Treatment Team.
Main duties of the job There are opportunities for someone with an enthusiasm for leadership to become involved in all other aspects of career development such as medical management and research.
As a training and teaching Trust there are opportunities to teach postgraduate psychiatric and GP trainees and undergraduates from Birmingham and Grenada Universities and newly formed links with Aston Medical School. Links to local Universities have always been strong and input to Birmingham and Wolverhampton Universities is welcomed.
Working for our organisation Black Country Older Adult Services
Older Adult Mental Health Services deliver multidisciplinary inpatient and community care for older adults with mental health conditions, underpinned by a person-centred and holistic model of care. Services operate across all four Black Country localities, with inpatient provision based in Walsall at the Dorothy Pattison Hospital Older Adult Unit, and at the newly refurbished Edward Street Hospital. These units provide specialist care for older adults with a range of mental health conditions, including functional illness and dementia. Exciting new developments include the Older Adult Hospital at Home Team and In-Reach to Care Home Team, enabling assessment and treatment in community settings and supporting admission avoidance where appropriate.
Detailed job description and main responsibilities The community mental health teams are based at Woodside and comprise of CPNs, Social Workers, Occupational Therapists and support workers. The team has introduced a single point of entry of referrals for the older Adult service.
The Central Team is one of 3 CMHTs providing services to the Dudley town.
The team consists of:
? 1 whole time equivalent (WTE) consultant psychiatrist ? 1 WTE specialty doctor/Clinical fellow ? 1 WTE medical secretary, Band 4 ? 5.2 WTE community psychiatric nurses, Band 6 ? Access to social worker ? Access to older adults senior occupational therapist and OT assistant ? Access to dedicated older adults psychologist to the Dudley locality ? 0.8 WTE support worker Band 3
The team expects to receive on average 5-7 new referrals a week and has in place a rapid assessment triaging service that is multi-disciplinary in nature, allowing assessment of up to 10 cases within 48 hours of receipt of referral.
It is expected that all team members (apart from the support workers) carry roughly equivalent numbers of cases as care co-ordinators.
The consultant psychiatrist is expected to carry a compact caseload of the most complex and unstable cases, but will also be available at short notice to provide consultation and advice to other team members, although they are not required to act as care co-ordinator. The successful candidate will be expected to provide cross-cover to other Older Adult Consultant colleagues. 2. Caseload numbers and support Based on the T&Cs of The New 2003 Consultant Contract, the programmed activities (PAs) would involve 7.5 direct clinical care PAs (DCC) and 2.5 supporting activities (SA) for a full time post. A whole time equivalent (WTE) Speciality or Equivalent Doctor and a WTE trainee (FY2, CT or GP VTS Trainee). Total population of Dudley borough is approximately 323,495 with 64,000 being 65 or over. The consultant has 3 clinics (3 PA) including home visits in a week, consisting of 4-5 patient in each clinic, the number of patients in each clinic are based on new or follow up patients.
They have small compact case load of complex patients and supported by care co-ordinators. The case load for this post is below 100. The team/consultant will have access to all standard services such as Psychology, Occupational Therapy, Community Psychiatric Nurses, Home Treatment/Crisis, Early Intervention and access to in-patients/respite beds.
A medical secretary will provide support to the post holder and the other medical staff in the Team. Office accommodation is available specific to the consultant, and there is IT support, both in terms of equipment and a department available for prompt advice and help if necessary. Clinical rooms are available at Bushey Fields Hospital for out-patient clinics. The consultant may also wish to see patients and families in other settings to provide a flexible service including at patient homes or at GP practices.
