Care Coordinator - Early Intervention

The closing date is 02 June 2026

Fixed term contract 6 months – maternity cover only. Secondment available.

1 full time post, 1 part time post (19hrs per week) – please state your preference in your application.

We are looking for a Nurse, Social Worker or Occupational Therapist to join us as a Care Coordinator (Band 6) in the Early Intervention Psychosis team in Peterborough – CAMEO.

CAMEO is the specialist Early Intervention Psychosis service in Cambridgeshire. This post will be located in the dynamic and multicultural city of Peterborough.

We cover Peterborough, Fenland and Huntingdon. We have adopted a hybrid model of working and would welcome applicants from neighboring counties.

* A multi‑disciplinary team including nurses, social workers, OTs, doctors, support workers, psychologists and CBTp therapists.

Main duties of the job

The role of the Care Coordinator is to work in partnership with CAMEO service users, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to CAMEO. Protocols as detailed in NICE guidance CG178 and EIPN Quality Standards CCQI 373 to include assertive outreach approaches, psycho‑education, social inclusion, recovery, care plans and discharge planning, working closely with partner agencies to facilitate this, and working alongside carers and families, offering them specialist interventions.

The post holder will promote the safety and wellbeing of service users and their families/carers.

The post holder will assess, plan and implement care, and provide specialist advice pertinent to their professional background and carry out relevant specialist interventions.

The post holder will provide clinical supervision to staff and students.

The post holder will ensure that the early intervention approach and ethos are adhered to, this includes working with the under‑17 age range as required.


Job responsibilities

To undertake assessment of individuals and their families/carers accessing the service with complex health presentations, including those service users presenting with higher levels of risk.

To develop alongside the service user, their care plan.

To offer specialist professional assessment and advice where required.

To assess and manage ongoing risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.

To regularly review risk factors and make changes to the management of them as necessary.

Travel independently around the county, meeting strict time deadlines. Must hold a full UK driving licence and have use of a vehicle. Public transport is not acceptable for this role. Please confirm in your application that you meet the specified criteria.


Person Specification


Education and Qualifications

* Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role OR Registered Social Worker OR Registered OT.
* Evidence of continuous professional development.


Experience

* Significant post‑qualifying experience in working with those presenting with serious mental illness.
* Experience of working alongside people with complex health problems in the designated care group.
* Experience of managing a caseload.
* Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems.
* Experience of conducting clinical assessments including risk assessments.
* Experience and knowledge of dual diagnosis/substance misuse.
* Previous experience of working in the community.
* Lived experience of mental health issues.


Skills & Abilities

* Excellent written and oral communication skills.
* Ability to develop effective professional relationships with others.
* Ability to develop own skills and knowledge and provide information to others to help their development.
* Skill of assessing and interpreting service user conditions with appropriate actions.
* Plan and organise complex activities or programmes requiring formulation and review.
* Able to use Word, PowerPoint, clinical systems and emails and/or willingness to learn.
* Ability to multitask and adapt mentally to different unpredictable situations.


Other

* Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.
* Punctual and flexible across hours of work when required.
* Demonstrate support for the values and beliefs of the Care Group and those of the Trust, especially the young person and their family's specific needs.


Physical Requirements

* Ability to use manual handling techniques.
* Energy to work with younger and older people.
* Monitor and maintain the health and safety of self and others in the working area.
* Required to use a PC.


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.


Salary

£39,959 to £48,117 per annum pro rata.
