Overview
The HMR Intermediate Tier Service is looking to recruit an enthusiastic and dynamic Band 6 Physiotherapist or Occupational Therapist to work within a multi-disciplinary team. The teams provide patient focused assessment and treatment for people within the Home in a Day Service. The Therapist will be expected to carry a caseload within the Home in a Day Service delivering assessments and treatments both in hospital and at home. This service was graded as 'outstanding' from the CQC.
Responsibilities
* To have responsibility for own caseload and deliver individualised assessments and interventions for patients with complex and changing needs
* To provide specialist support and education to therapists and wider teams
* To provide line management for junior members of the therapy team
Qualifications
* Essential: HCPC registration in Occupational Therapy or Physiotherapy
* Essential: Degree or diploma in Occupational Therapy
* Desirable: Clinical Educators accreditation
Knowledge, Training & Experience
* Involvement in and responsibility for service development activity
* Enable patient choice and participation and support others to do so
* Experience of liaison with other professionals and partnership working
* Post graduate experience in acute medical wards and complex discharge planning
* Specialist knowledge of anatomy and physiology to underpin relevant clinical skills and clinical reasoning
Skills & Abilities
* Excellent organisational skills including time management, prioritising, and the ability to complete a task
* Ability to use advanced clinical reasoning, making professional judgements in complex and challenging situations
* Able to travel across sites and community to meet patient need, access training and attend meetings
* Ability to work over 7 days
* Effective computer skills
* Excellent written and verbal communication skills
