Join and develop our team of PCN Care Coordinators working across Chesterfield and Dronfield, working 30 hours per week. This role involves working innovatively across a wide range of work streams including Learning Disabilities, Ageing Well, and the Pharmacy team, in collaboration with practices and the multidisciplinary team (MDT) within the PCN to deliver high-quality primary health care.
You will be self-motivated, able to work autonomously, and have excellent written and verbal communication skills. You will be a strategic thinker who can collaborate across several multidisciplinary networks to fulfil the Chesterfield and Dronfield PCN vision. Experience in General Practice is ideal, and you may have completed the NHS 2-day National requirement personalised accredited care coordination course. The role requires flexibility and the ability to adapt to the rapidly changing environment of primary care. The service operates between 8:00–18:00, so applicants must be available to work within these hours. Please note we are not an Agenda for Change Organisation. Interviews will be held face to face on the afternoon of 6 May.
Main Responsibilities
* Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care.
* Work with the Advanced Wellbeing Team (AWT) to identify and manage a caseload of patients and refer to other health professionals within the PCN where appropriate.
* Develop excellent working relationships with key stakeholders and coordinate care with multi-agencies.
* Act as an intermediary between individuals, receptionists, administrators, clinicians, social workers, therapists and mental health teams.
* Keep SystmOne/EMIS up to date with relevant records for colleagues involved in care; maintain an up-to-date portfolio of reflections and case management stories; operate under supervision of a Lead Clinician.
* Ensure a point of contact is available to case-managed or potential case-managed individuals during working hours; communicate effectively with service users, their families/carers, staff, and the public.
* Refer and identify complex cases following processes with Leads GPs; support efforts to reduce inequalities and unwarranted variation in health care.
* Manage and prioritise workload daily, coordinate MDT/CST meetings, take minutes, and chase progress on actions identified in meetings.
* Advise and support patients and carers to make informed decisions, using appropriate decision-making aids; coordinate and manage personalised care and support plans (PCSP).
* Coordinate a designated caseload with clinicians to determine appropriate interventions and maintain a fluid caseload through supervision.
* Follow safeguarding policies when concerns arise; signpost to relevant services and work with social prescribing link workers and other primary care professionals.
* Contribute to assessments to identify needs to maintain independence at home; ensure information is accessible in written or verbal form.
* Engage with patient participation groups as part of PCN community engagement activities; use IT tools (Microsoft Office, Outlook, Excel) effectively.
* Perform general administrative duties to support the role and other reasonable duties as requested by a manager.
