An opportunity has arisen for a highly motivated individual with relevant knowledge and experience of providing housing and support to vulnerable people who require help to live independently and prevent homelessness.
The role:
In this post you will be working as part of the Floating Support Service Team in Housing Management.
As part of the role, you will complete Assessment Forms in response to referrals requesting the Floating Support Service and you will advise and offer practical support and advice to achieve independent living, ideally minimising any risks due to an individual’s vulnerability.
You will work with the service user in setting up a support plan and carrying out regular reviews with the aim of the vulnerable person successfully completing a programme of support. You will look at the support options available to meet the need of the individual, enabling them to maintain their accommodation and overall independence.
There will be the opportunity to liaise and work in partnership with statutory bodies and agencies to ensure a successful outcome for the individual.
About you:
You will need to have proven skills and expertise to assess and advise on a variety of support and practical options that may be available to vulnerable people, which helps them to maintain their home and wellbeing.
A sound understanding of working with people with complex needs as well as skills and knowledge of meeting support needs will be of great benefit in this exciting role.
