Role summary
Area East Continuing Healthcare Care Team have a unique opportunity for an enthusiastic, motivated RMN for the role of Continuing Health Care Lead Nurse OPMH Band 7. The role involves managing a team of Nurse Reviewers who visit patients in care homes and in their own homes to review and assess their needs in relation to NHS Funding, in accordance with National guidance. The post holder would work closely with the Discharge teams across hospital sites, Nursing Homes, Social Services departments, and other NHS professionals, including contract teams.
Responsibilities
* Coordinate, scrutinise CHC applications and participate in CHC and FNC reviews; attend and participate in Multi-Disciplinary Team (MDT) meetings within the area/department where required.
* Provide training packages for Multi-Agency staff on CHC issues and promote good practice.
* Support the management duties of the CHC team locally and oversee the local resolution process within the Area, including meeting and liaising with families to discuss panel outcomes when individuals challenge eligibility decisions.
* Manage the local resolution process within the Area for Inter-Agency Disputes, including liaising with the appropriate Local Authority Team Manager in disputed CHC eligibility cases.
* Collaborate with statutory agencies where safeguarding issues are identified and attend POVA/Strategy meetings as required.
* Work with CMHTs, DN Services, and Discharge liaison Teams to ensure safe and timely discharge/ transfer of patients.
* Implement and monitor Continuing NHS Health Care funding (CHC) in Care Homes and Domiciliary care packages in line with the national CHC framework and NHS Funded Nursing Care (FNC).
* Extensive joint working with Local Authorities to ensure high-quality and safe service delivery for patients in the Independent Sector funded by BCUHB.
* Provide training for Multi-Agency staff on CHC.
Qualifications and requirements
* Registered Mental Health Nurse (RMN) with appropriate qualification and registration.
* Knowledge of the Continuing Health Care Framework and experience in assessing patients for continuing health care.
* Experience in coordinating complex care funding processes and working within MDT settings.
* Ability to communicate effectively with families, care providers, and partner agencies; sensitively handle confidential information.
* Welsh language ability is desirable; applications from English and Welsh speakers are welcome.
