We are seeking an enthusiastic and experienced Paramedic to join our friendly and supportive GP practice team. This is an exciting opportunity to work within primary care, delivering high-quality acute same day care and supporting our dedicated home visiting service as part of a team of three Paramedics.

The successful candidate will play a key role in assessing, treating, and managing patients with urgent but non-emergency clinical needs, helping to improve access to care and support continuity for our patient population.


Main duties of the job

* Provide autonomous clinical assessment, diagnosis, and treatment for patients presenting with acute same day problems.
* Undertake home visits for housebound and vulnerable patients as part of the practice visiting team.
* Work collaboratively with GPs, nurses, reception staff, pharmacists, and the wider multidisciplinary team.
* Manage a varied caseload safely and effectively within scope of practice.
* Support care planning, admission avoidance, and appropriate onward referrals.
* Maintain accurate and contemporaneous clinical records using the practice clinical system.
* Contribute to service development and quality improvement initiatives within the practice.

Sunbury Health Centre Group Practice is a well-established, forward-thinking training practice providing high-quality care to a growing population of approximately 19,300 patients in a pleasant Surrey location with excellent transport links.

Our experienced clinical team includes 6 GP Partners, 6 Salaried GPs, GP Registrars, Prescribing ANPs, Paramedics, Clinical Pharmacists & Practice Nurses. We have a team of HCAs & Phlebotomists alongside a highly skilled and approachable management and administrative team.

Job responsibilities

Key duties and responsibilities

Assess and triage patients, including same daytriage, and as appropriate provide definitive treatment or make necessaryreferrals to other members of the practice team

Advise patients on general healthcare andpromote self-management where appropriate, including signposting patients toother community or voluntary services.

Participate in all treatment and preventativehealthcare services in the Practice as delegated and agreed by the supervisingGP(s).

Provide direct clinical care to patients usingestablished clinical guidelines.

Consult and refer patients to physicians,medical specialists and other health professionals as indicated.

Interview patients, take medical histories,perform physical examinations, analyse, diagnose and explain medical problemsduring home visits.

Perform specialist health checks and reviews

Perform investigatory procedures as required

Undertake the collection of pathologicalspecimens including intravenous blood samples, swabs, etc.

Support the delivery of anticipatory care plansand lead certain community services (e.g. monitoring blood pressure anddiabetes risk of elderly patients living in sheltered housing)

Communicate proactively and effectively with allcolleagues across the multi-disciplinary team, attending and contributing tomeetings as required

Maintain accurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient health care and registration with the practice

Work with patients in order to supportcompliance with and adherence to prescribed treatments

Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with an undifferentiateddiagnosis, and patients with complex needs

Clinically examine and assess patient needs froma physiological and psychological perspective, and plan clinical careaccordingly

Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care

Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based treatment methods into amanagement plan

Support patients to adopt health promotionstrategies that promote healthy lifestyles, and apply principles of self-care

Help work on practice QOF domains and someselected chronic disease targets

Quality Requirements

Enhance own performance through continuousprofessional development, keep up to date withcurrent evidence-based practice, and impart own knowledge and skills tocolleagues to meet the needs of the service

Recognise and workwithin own competence and professional code of conduct as regulated by the HCPCand College of Paramedics

Assess effectivenessof care delivery through self and peer review, benchmarking and formalevaluation

In partnership withother clinical teams, collaborate on improving the quality of health careresponding to local and national policies and initiatives as appropriate

Administrative requirements

Produce accurate, contemporaneous and completerecords of patient consultation, consistent with legislation, policies andprocedures

Be aware of data protection (GDPR) andconfidentiality issues

Use technology and appropriate software as anaid to management in planning, implementation and monitoring of care,presenting and communicating information

Review and process data using accurate readcodes in order to ensure easy and accurate information retrieval for monitoringand audit processes.

Supervision

The post holder will have access to appropriateclinical supervision and an appropriate named individual in the practice to providegeneral advice and support on a day to day basis.

Confidentiality:

In the course of seeking treatment, patientsentrust us with, or allow us to gather, sensitive information in relation totheir health and other matters. They doso in confidence and have the right to expect that staff will respect theirprivacy and act appropriately

In the performance of the duties outlined inthis job description, the post-holder may have access to confidentialinformation relating to patients and their carers, practice staff and otherhealthcare workers. They may also haveaccess to information relating to the practice as a businessorganisation. All such information from any source is to be regarded asstrictly confidential

Information relating to patients, carers,colleagues, other healthcare workers or the business of the practice may onlybe divulged to authorised persons in accordance with the practice policies andprocedures relating to confidentiality and the protection of personal andsensitive data

Health & safety:

The post-holder will assist in promoting andmaintaining their own and others health, safety and security as defined in thepractice Health & Safety policy, the practice Health & Safety manual,and the practice Infection Control policy and published procedures. This willinclude:

Using personal security systems within theworkplace according to practice guidelines

Identifying the risks involved in workactivities and undertaking such activities in a way that manages those risks

Making effective use of training to updateknowledge and skills

Using appropriate infection control procedures,maintaining work areas in a tidy and safe way and free from hazards

Actively reporting of health and safety hazardsand infection hazards immediately when recognised

Keeping own work areas and general / patientareas generally clean, assisting in the maintenance of general standards ofcleanliness consistent with the scope of the job holders role

Undertaking periodic infection control training(minimum annually)

Demonstrate due regard for safeguarding andpromoting the welfare of children.

Person Specification


Experience

* We are looking for a motivated clinician who:
* Is a qualified Paramedic registered with the Health and Care Professions Council (HCPC).
* Has experience in urgent care, community care, primary care, or emergency medicine.
* Demonstrates excellent communication and clinical decision-making skills.
* Is confident working independently while also valuing teamwork.
* Has a compassionate, patient-centred approach.
* Holds a full UK driving licence.
* Experience working in Primary Care, aware of common chronic and acute illnesses in a GP setting
* Evidence of working across organizational boundaries within health and social care.
* Independent thinker with good judgement, problem-solving and analytical skills.
* Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences


Qualifications

* BSc in a training programme approved by the College of Paramedics.
* Health & Care Professions Council (HCPC) registration.
* Able to operate at an advanced level of clinical practice.
* Full UK driving license.
* Pre-reg MSc in a training programme approved by the College of Paramedics.
