The Hospital at Home (H@H) service based at Lorn and the Isles Hospital covers the Oban and Lorn area. It is a developing service that has proven to be a safe alternative to hospital admission. The service provides acute care at home with the objective of reducing hospital admission and reducing length of stay in hospital through expanding clinical pathways. The Band 5 Staff Nurse post within the H@H service is a new role within the Hospital at Home team.
The post is a development post with a supported educational element to enhance clinical skills and clinical decision making skills within the
multidisciplinary team. This will enhance skills in management of patients with both acute and long term conditions across a number of different clinical pathways, where their medical conditions can be managed safely and effectively at home.
