Position Summary:
Commonwealth Care Alliance’s (CCA) Palliative Care Team, comprised of multidisciplinary clinicians, provides care consultation and delivery on-demand, along with scheduled non-urgent services to dually eligible CCA members with complex medical, behavioral, and social needs requiring intensive support.
The team includes nursing (RN), advanced practice (NP), and Behavioral Health (LICSW) team members. The Palliative RN reports to the Palliative Manager and works within the interdisciplinary team to evaluate and develop palliative management plans for patients needing goals of care conversations, experiencing acute events, or nearing end of life. Responsibilities include episodic serious disease management, urgent visits, promoting preventive care, pain management, and supporting end-of-life care.
The role involves collaborating with other team members and external providers to adjust care plans, identify needs for equipment and services, and plan for end-of-life location. The RN also provides telehealth clinical care, educates community partners, and participates in quality improvement efforts and organizational committees. Mentorship to new employees is also expected.
Essential Duties include:
* Performing urgent and scheduled visits for symptom management and disease education.
* Delivering palliative care according to CCA guidelines based on patient goals.
* Follow-up care, documentation, and adjusting care plans as needed.
* Providing end-of-life care in coordination with care partners.
* Utilizing telehealth technologies for clinical services.
* Participating in organizational improvement initiatives and mentorship.
Working conditions require travel across MA, a valid driver’s license, and physical ability to perform job duties. Education requirements include an Associate Degree in Nursing; a Bachelor’s or Master’s degree is preferred. Licensing as an RN in Massachusetts is mandatory. At least 3 years of experience in medical-surgical, VNA, or hospice settings is required, with experience serving complex Medicare/Medicaid populations preferred. Skills include clinical competence, excellent communication, mentorship, adaptability, and fluency in English; additional languages are a plus.
