Integrated Urgent Care Triage Nurse

Overview
It is expected to be 12h shifts (8am-8pm), and we encourage positive work‑life balance. The role involves triaging incoming referrals, assessing patients' needs, determining priority of care and processing referrals accordingly while contributing to the delivery of high‑standard health care. The nurse will oversee and respond to remote monitoring systems supported by the senior clinical team and will communicate regularly with patients and carers.
Main duties of the job
 * Process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
 * Document triage notes to ensure the patient’s record is complete and fit for audit purposes.
 * Escalate concerns when appropriate by liaising with the clinical teams in the community and the integrated team.
 * Assess, prescribe and arrange care interventions and assessments for all patients referred to the Single Point of Access (UCCH, 2h‑UCR, Rapid Response and Virtual Ward).
 * Work within the integrated team to prevent unnecessary admission to hospital by utilising all available pathways.
 * Support clinical activity in the Virtual Ward Hub and be competent in all aspects of that hub, interchanging role with the VW triage nurse when activity increases.
 * Be responsible for clinical activity in UCCH and in the NCHC SPoC as 2h‑UCR, demonstrating competence in all aspects of UCR triaging and allocation.
Job responsibilities
 * Assess, plan, implement and evaluate all aspects of patient care, and develop care plans using clinical reasoning skills.
 * Communicate complex information to patients and carers about assessment, diagnosis, prognosis and treatment plan.
 * Liaise with GPs, social services, inpatient teams, allied health professionals and the voluntary sector to ensure identified needs are met and care coordinated.
 * Utilise the remote monitoring facility for Virtual Ward patients, understand escalation points and seek further clinical support when required.
 * Be accountable for work delegated to others within the team.
 * Ensure daily completion of data entry in Cleo, SystmOne (or other patient records used by the Service).
 * Adhere to universal precautions and infection control policies when exposed to bodily fluids, infected material or blood products.
Person Specification
 * Registered Adult Nurse qualification.
 * Diploma, BSc or higher.
 * Current NMC registration.
 * Evidence of continuing professional development.
Experience
 * Experience working within a multidisciplinary team.
 * Experience working with patients with multiple pathologies and delivering care plans to meet their needs.
 * Experience working within a community setting.
 * Knowledge of SystmOne.
 * Experience working within an acute or pre‑hospital setting.

 * Be able to make own travel arrangements to patients, clinics, base and meetings as required.
 * The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary. Some roles require additional level of checks as part of the employment process, i.e. roles which come into contact with children, vulnerable adults or their families.
 * This is a 12h a day, 7 day a week service and some weekend working will be expected. Hours of service delivery, 08:00‑20:00.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and therefore it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary
£31,049 to £37,796 a year per annum pro rata
