Overview

Local Analgesia Administration Certificate

An opportunity has arisen for a Band 6 Podiatrist to join our dynamic team. The role involves timely assessment and treatment of a wide range of conditions using specialist skills and provision of specialist advice. The post involves collaborative working with Acute Services including James Paget University Hospital and Norfolk and Norwich Hospital. The post is based in a clinical setting; the postholder will carry and be responsible for their own caseload, assess, diagnose, and develop and implement individualised care programmes for patients with complex podiatry presentations including acute and chronic pain, significant physical impairments due to pain or injury and psychological issues. The postholder will maintain their own records as an autonomous practitioner and work with Clinical Leads to develop ECCH\'s integrated Podiatry Service in line with evidence-based practice.

Main duties of the job

* To carry and be responsible for own caseload. Assess, diagnose, develop and implement individualised care programmes for a caseload of patients who have complex Podiatry presentations including acute and chronic pain patterns; significant physical impairments due to pain or injury and psychological issues and maintain own records as an autonomous practitioner.
* To be a source of expertise for ECCH\'s integrated Podiatry service, facilitating an efficient and effective care pathway for patients.
* Access to arrange imaging, as appropriate, either independently or through our MAT\'s service/leads.
* Developing individualised care and treatment programmes with suitable progression in line with patient expectation and ability.
* Ensure effective communication to enable patients to achieve their maximum potential for self-management, particularly where this is impaired due to pain and/or fear.
* A high standard of communication skills and clinical work is required, which will include teaching and supervision of other health professionals, non-registered staff and students.
* Work with the Clinical Leads for Podiatry to develop the service in line with evidence-based clinical best practice.

Job Description and Responsibilities

Clinical Duties:

* To be responsible for the assessment, diagnosis, planning, implementation and evaluation of podiatric care for a variety of patients with a wide range of clinical needs including acute conditions such as ingrowing toenails, high risk diabetes, chronic neuropathy, vascular problems, chronic diseases (e.g. rheumatoid arthritis), wound management/tissue viability, and terminally ill patients.
* Post-registration experience with a wide range of clinical problems and, ideally, completion of a Band 6 competency-based programme.
* Care will be provided in a range of primary and secondary care settings, including hospital clinics, hospital wards, health centre community clinics, residential homes for the elderly and patients\' homes.
* Be responsible for the assessment and diagnosis of new patients with subsequent production of individual care packages and provision of appropriate footcare education to groups of patients.
* Maintain highly developed physical skills and precision hand/eye coordination for prolonged periods.
* Identify when patients require intervention from other professionals by following agreed referral pathways (e.g., acute wound care/biomechanics).
* Participate in the departmental rota for nail surgery and perform diabetic assessment reviews in community clinics (diabetic foot assessment with Doppler, monofilament, tuning fork). Identify risk factors and categorise patients into appropriate risk bands; refer to specialist services as required and liaise with other health care professionals to enable optimum care.
* Plan day-to-day workloads for home visits and visits to residential homes for the older person; provide foot health education to patients, relatives and carers; participate in departmental policies and procedures; maintain stock security and undertake relevant health and safety duties.
* Follow departmental policies and guidelines while applying professional judgment within ethical boundaries; move and handle immobile patients safely; provide cover for other podiatrists at short notice; liaise with GPs or consultants to arrange interventions as necessary (e.g., antibiotic cover, referral to A&E).
* Manufacture simple chair-side orthoses and produce templates and foot casts for appliance manufacture.


Communication

* Work effectively as a member of the multi-professional team and liaise with agencies such as social services and hospitals.
* Maintain accurate records of all patient consultations and attend departmental meetings; ensure confidentiality of information and effectively communicate with patients, relatives and carers.


Health and Safety

* Report accidents, complaints, equipment defects, near misses and untoward incidents; comply with health and safety legislation including COSHH and infection control; attend mandatory training.


Research and Development/Audit

* Participate in audits and keep up to date with evidence-based practice; be involved in research and development and clinical governance initiatives.


Professional Development

* Maintain HCPC registration and work within the HPC code of conduct; participate in continuing professional development and performance reviews; supervise assistants and Band 5 podiatrists where appropriate; mentor students and participate in in-service training;
* Contribute to sharing knowledge and demonstrate ECCH values and behaviours (CARE: Compassion, Action, Respect, Everyone).


Person Specification

Essential

Qualifications: Registration with HCPC; Podiatry Diploma/Degree; Local Analgesia Administration Certificate; post-registration experience including dealing with the at-risk foot and completion of Band 6 competency programme.

Desirable

IT skills; SystmOne experience.

Experience

Essential: Previous post-registration experience; experience managing a caseload in team and isolated settings; NHS student placement. Desirable: involvement in health education packages and departmental protocols.

* Good verbal and written communication skills; good organisational skills; ability to manage a high-risk caseload; ability to use vascular and sensory equipment; ability to produce, document, implement and evaluate care plans; able to cope with unpredictable work patterns; ability to prescribe orthoses and assess/evaluate footwear.
