One of my local government client is seeking a dedicated Rough Sleeper Outreach Worker to contribute to the Council’s aim of ensuring rough sleeping is rare, brief and non-recurring. This role supports vulnerable individuals through targeted street outreach, early intervention, personalised support planning and multi-agency partnership work.
Key Responsibilities:

Deliver an intelligence-led, flexible and assertive street outreach service to identify, support and engage rough sleepers.

Provide intensive, person-centred support, helping individuals access accommodation, treatment, and other relevant services.
*
Complete comprehensive assessments, risk assessments, and detailed support plans for individuals on the rough sleeping pathway and in temporary accommodation.
*
Support individuals in developing independent living skills, tenancy sustainment and progression into long-term housing.
*
Work closely with Housing Options, Adult Social Care, health partners, emergency services and voluntary organisations to coordinate interventions.
*
Lead or participate in crisis intervention, safeguarding processes and multi-agency case conferences.
*
Prevent homelessness by building partnerships with hospitals, prisons, landlords and supported housing services.
*
Assist with the annual rough sleeper count and Severe Weather Emergency Protocol (SWEP) delivery.
*
Maintain accurate and detailed case records on Council systems.
*
Act as an advocate for rough sleepers and provide a single point of contact for partner organisations.
Requirements:
*
Experience working or volunteering in a housing, homelessness, care or support environment.
*
Strong understanding of homelessness, rough sleeping, welfare benefits and barriers faced by vulnerable individuals.
*
Experience delivering assessments, support plans and key working.
*
Ability to manage challenging behaviour, work under pressure and prioritise caseloads.
*
Excellent communication, interpersonal and negotiation skills.
*
Ability to work collaboratively within a multi-agency environment.
*
IT literacy including Microsoft Office, case management systems and accurate record-keeping.
*
Willingness to work outside normal office hours for street outreach.
*
Full driving licence and access to own vehicle.
*
DBS clearance required.
