Community Care Services Coordinator- Outreach

We are seeking ahighlymotivated Leader to Coordinate an extension to our Supported Living servicewithin the community. This is a fantastic opportunity to make a realdifference in the lives of the people we support, while driving quality andinnovation across our services.


Main duties of the job

Aboutthe role

Coordinate support for two young adults living intheir own home, ensuring the staff rota is in line with their needs.

Keep up to date and review relevant support plans,risk assessments and tools to support the staff team to deliver high qualitysupport.

Liaise with families, professionals and key peoplewith regards to day-to-day queries and updates.

Undertake regular quality assurance and gainfeedback from staff, families and people we support.

To work with the people we support as and whenrequired in order to build positive relationships, guide the team and be apresent leader.

Job responsibilities

Purpose of the Job:

To Coordinate and facilitate service provision supporting people who have been

assessed of having a need for support, promoting their independence and enabling

choice and control.

To promote social inclusion, reducing isolation and contribute to service

development to meet both individuals and service outcomes.


Person Specification


Skills

* Able to manage health and safety issues through completion of
* the risk assessment processes
* Ability to liaise with other agencies, organisations, clients and
* their carers
* Motivated and committed
* Able to work on own initiative and as part of a team
* Tolerant and diplomatic
* Adaptable and flexible
* Positive attitude to disability and an understanding of the purpose
* of advocacy
* Appreciation of a multi-agency approach
* Ability to prioritise own and others work and motivate staff
* members
* Able to manage own time and that of others
* Ability to demonstrate practical interpersonal skills
* Ability to receive and communicate information verbally and in
* writing
* Able to work with confidentiality, sensitivity and diplomacy
* Supervisory skills
* Aptitude with IT


Qualifications

* NVQ 3/QCF in Social Care or willingness and ability to achieve
* within agreed timescale
* Assessors award or willingness to achieve within agreed
* timescale


Experience

* Knowledge of Person Centred Planning
* Previous experience in a similar role
* Experience of community development work
* Experience of leading and supporting a small staff team
